What issue do you have?
I am new to joplin and tested image attachment storage.
I find that no matter I delete image markdown content in text or delete entire note, the pasted images will become orphaned.
I am sure I emptied my trash bin so the resource should be deleted because they won't be recovered.
reproduce steps:
- add new note
- paste image to note
- right click on the image in md viewer, reveal in finder
- delete md content and see resource folder
- delete note & empty trash bin & check resource folder

At present, how long attachments are kept after being unlinked from all notes is determined by the "Keep note history for" setting.

Though I think this is a bug because you don't have any chance to find back the orphan contents if you've emptied trash.
